ACP makes recommendations to improve transitions between health care settings

Challenges with transitioning patients from one health care setting to another can result in significant health implications and financial costs, says the American College of Physicians (ACP) in a new policy paper published today. “Beyond the Discharge: Principles of Effective Care Transitions Between Settings: An American College of Physicians Policy Position Paper,” makes recommendations for more successful care transitions that can improve patient safety, health care outcomes, and patient and caregiver satisfaction.
